AMENDED ORDER

This is a verified petition dated July 23, 2013 filed by the petitioner through counsel for the connection of entries in the certificate of live birth of John Rhey Zulueta Pascual with the City Civil Registrar of San Carlos City, Pangasinan.

Finding the petition to be sufficient in form and substance, the initial hearing of this case, is hereby set on October 30, 2013 at 8:30 in the morning, at which time, date and place, all persons having any  opposition thereto may appear and show cause, if any, why the petition should not be granted.

Let a copy of this Order be published at the expense of the petitioner once a week for three (3) consecutive weeks in a newspaper of general circulation in the Province of Pangasinan including the Cities of Dagupan, San Carlos, Urdaneta and Alaminos, which shall be selected on the basis of raffle.

Furnish a copy of this Order to the Offices of the City Civil Registrar of San Carlos City, Pangasinan, the Solicitor General, Makati City and the Office of the City Prosecutor, San Carlos City, Pangasinan.
