Dagupan City launches online vaccine registration
THE Dagupan City government launched its online vaccination website last week, May 19, for the vaccination of the A3 priority group – adults with comorbidities – residing in the city.
Residents who registered on the website, vaccine.dagupan.gov.ph, will receive confirmation of their successful registration via text message and will be advised of their vaccination schedule by the City Health Office (CHO).
Health workers (A1) and senior citizens (A2) may also register for their schedule.
Meanwhile, persons who have completed their first and second doses will be issued a COVID-19 vaccination card by the city which may be presented as proof.
The city-issued vaccination card contains the person’s name, address, ID number, and date of birth in front, details of vaccine used, batch number, dates of first and second doses, and a QR code that directs to the city’s vaccine website, confirming the card holder’s information.
Persons that have not received their second dose, a temporary vaccination card will be given.
The Dagupan City COVID-19 vaccination card is recognized by the Department of Health.
As of May 7, 2021, the city government already inoculated 11,300 residents in both A1 and A2 categories. (Ahikam Pasion)
