San Carlos City activates online vax registration

THE online vaccine registration developed by the San Carlos City government for its residents was recently activated to determine the vaccine acceptability of San Carlenians.

City Information Officer Jhuliano Nazareno said applicants can access the link of the Google form at the Facebook page of San Carlos City Information Office or scan the quick response (QR) code and log-in with a Gmail account.

Residents will be asked to provide information and attach a picture or documents that would prove your qualification to the priority group.

Nazareno said that all submissions will be added to their master list and the copy will be forwarded to the Department of Information and Communications Technology and Department of Health.

 The DOH will send vaccine allocation based on list in priority list.

He said applicants will receive a text message confirming their date and location for their scheduled inoculation.

Meanwhile, those who fail to register online can still do so in physical mall-based vaccine registration site in the city.  However, the scheduled opening of mall-based registration was postponed to end of May or first week of June this year.

The 18,000 senior citizens in the city are next in line for the vaccination program. (Jerick Pasiliao)
