This is a verified petition for Correction of Entries in the Certificate of Live Birth of Bernalisa Garcia Degorio and Marriage Contract of Rey J. Dolor and Bernaliza G. Degorio, alleging among others that the petitioner Bernaliza Garcia Degorio is of legal age, Filipino and a resident of Brgy. Macatiw, Alaminos City, Pangasinan, while the respondent is the City Civil Registrar of Alaminos City, Pangasinan holding office at the City Hall of Alaminos City, Pangasinan where it may be served with summons and other court processes; that the petitioner is the person whose Certificate of Live Birth and Certificate of Marriage are to be corrected; that the petitioner recently obtained her Certificate of Live Birth and Marriage Contract copy of which are hereto attached as Annexes “A” and “B” wherein she discovered erroneous entries; that said erroneous entries discovered by the petitioner must be corrected to read as follows:

MARRIAGE CONTRACT OF REY J. DOLOR and BERNALIZA G. DE GORIO

Name of Mother from: FLORDELIZA GARCIA to: FORTALIZA V. GARCIA

Person Who Gave Consent or Advice from : FLORDELIZA GARCIA to: FORTALIZA V. GARCIA

Let the initial hearing be set on January 28, 2015 at 8:30 o’clock in the morning at which time, date and place any interested person may appear and show cause why said petition should not be granted.

Let this Order be published at the expense of the petitioner once a week for three (3) consecutive weeks in a newspaper of general circulation in the Province of Pangasinan including the cities of Dagupan, San Carlos, Urdaneta and Alaminos.

Likewise, let copies of this Order, petition and annexes be furnished the Hon. Solicitor General at 134 Amorsolo St., Legaspi Village, Makati City.

Furnish copy of this Order to parties and counsels accordingly.
