Notice is hereby given that a estate of the late MARIO P. CALUGAY, who died on June 28, 2014, consisting of a Deposit at Bank of the Philippine Islands/BPI Family Bank (Savings Account No. 8556-0870-56, located at Lucao Branch, with the amount of P97,449.65 as of June 28, 2014, is the subject of extrajudicial settlement among heirs with waiver of rights in favor of CORNELIA B. CALUGAY, who is hereby empowered to withdraw the total amount from BPI, under Doc. No. 71; Pages 16; Book No. IV; Series of 2014, under Notary Public Atty. Rafael T. Martinez.
