A verified petition was filed by Trinidad M. Andrion, praying for the correction of entry in  her certificate  of live birth in the Office of the Local Civil Registrar of Urdaneta City, Pangasinan, as follows:

Finding the petition to be sufficient in form and substance, the same is set for hearing on AUGUST 06, 2014 at 8:30 o’clock in the morning. Any interested person may appear and show cause, if any, why this petition should not be granted.

Let copy of this Order be published ONCE a week for three (3) consecutive weeks in the “SUNDAY PUNCH” at the expense of the petitioner.

Furnish the Office of the Solicitor General, the Office of the Local Civil Registrar, Urdaneta City, Pangasinan, the Office of the City Prosecutor, Urdaneta City, Pangasinan copies of this Order, the Petition and its Annexes.
