Notice is hereby given that the estate left by the late DIONISIO D. SAPLAN, SR., who died on Dec. 3, 2013, consisting of a Savings Deposit Account with the Philippine Veterans Bank- Dagupan City Branch under Savings Account No. 0020-215202-101 with a balance of P141,772.68, is the subject of affidavit of claim with waiver of rights in favor of DIONISIO R. SAPLAN JR.
