ORDER

This is a verified petition praying for the cancellation/correction of the erroneous entries in the petitioner’s Certificate of Live Birth, particularly as follows; to wit:

a) Date & Place of Marriage of Parents

From: October 15, 1998-Caloocan City Hall, Caloocan City, Metro Manila

To: Not applicable;

b) Name of the child: From: Charles Darwin Paglingayen Cruz

To: Charles Darwin Paglingayen; and

c) Informant- From: Ada P. Cruz

To: Ada M. Paglingayen

Finding the petition to be sufficient, the initial hearing is hereby set on February 3, 2014 at 8:30 o’clock in the morning, at which time, date and place, all persons having any opposition thereto may appear and show cause, if any, why the petition should not be granted.

Let a copy of this Order be published at the expense of the petitioner, once a week for three (3) consecutive weeks in a newspaper of general circulation in the Province of Pangasinan, including the cities of San Carlos, Dagupan, Alaminos and Urdaneta, which shall be selected  on the basis of a raffle.

Furnish copy of this Order to the Local Civil Registrar, San Carlos City, Pangasinan, the Solicitor General, Makati City and the City Prosecutor, San Carlos City, Pangasinan.
