A verified petition was filed by Felimon Bibat Alfonso praying for the correction of entry in his Certificate of Live Birth in the Office of the Local Civil Registrar of San Manuel, Pangasinan, as follow:

Finding the petition to be sufficient in form and substance, the same is set for hearing on OCTOBER 30, 2013 at 8:30 o’clock in the morning. Any interested person may appear and show cause, if any, why this petition should not be granted.

Let copy of this Order be published ONCE a week for three (3) consecutive weeks in the “SUNDAY PUNCH” at the expense of the petitioners.

Furnish the Office of the Solicitor General, the Office of the Local Civil Registrar, San Manuel, Pangasinan, the Office of the Provincial Prosecutor, Urdaneta City, Pangasinan copies of this Order, the Petition and its Annexes.
