ORDER

This is a verified petition filed by Kim Jamaica M. Dumalanta, represented by her mother Amalia Meneses Mabanglo, praying for the correction of the erroneous entries in the record of her birth particularly, the following:

1. Sex – From: Male

To: Female

2. Date & Place of Marriage of Parents – From: November 2, 1992, San Carlos City, Pangasinan To: Not married.

Finding the petition to be sufficient in form and substance, initial hearing thereof is hereby set on August 28, 2012 at 8:30 o’clock in the morning, at which time, date and place, all persons having any opposition thereto may appear and show cause, if any, why the petition should not be granted.

Let this Order be published at the expense of the petitioner, once a week for three (3) consecutive weeks in a newspaper of general circulation in the Province of Pangasinan, including the cities of Dagupan, Alaminos, Urdaneta and San Carlos, which shall be selected on the basis of a raffle.

Likewise, let copies of this Order be furnished each to the Local Civil Registrar of San Carlos City, Pangasinan; the Solicitor General of the Philippines Makati City; and the Provincial Prosecutor’s Office, San Carlos City, Pangasinan.
