ORDER

Petitioner’s verified Petition seeks to correct entries in here Certificate of Live Birth, praying that the following entries in the same be corrected to read as follows:

a) Under Entry No. 1, petitioner’s last name: From “Galman” to “Galvan”;

b) Under Entry No. 2, Sex of the petitioner: From “MALE” to “FEMALE”;

c) Under Entry No. 6, Maiden Middle Name of petitioner’s mother should be entered as “De Guzman”

d) Under Entry No. 9, Middle Name of petitioner’s father should be entered as “Delos Santos” and;

e) Under Entry No. 9, The Last Name of her father should be “Galvan” and not “Galman”;

The Court hereby sets the initial hearing of this petition on May 29, 2012 at 1:30 o’clock in the afternoon, at which date, time and place, any interested party may appear and show cause why the petition shall not be granted.

WHEREFORE, let a copy of this Order be published once a week for three (3) consecutive weeks in the Sunday Punch a newspaper of general circulation in the Province of Pangasinan and its cities.

Furnish copies of this Order to the Office of the Solicitor General, Makati City, the Municipal  Civil Registrar of Labrador, Pangasinan and the Office of the Provincial Prosecutor, Lingayen, Pangasinan.
