MENDOZA,  who died on Nov. 16, 2009 consisting of a Savings Deposit Account with the Philippine Veterans Bank-Dagupan Branch under Savings Account No. 5002002161 with a balance of P127,492.04, is the subject of Affidavit of Claim with waiver of rights in favor of RUFINA M. PERALTA.
