ORDER
Petitioner seeks the correction of entries in her record of birth with the Local Civil Registrar of Mangatarem, Pangasinan, specifically, as to the name of the child’s mother from “Peregrina Aguilar” to “Melva A. Vegilla, and the Cancellation of the name “Jose Vegilla” and other entries pertaining to Child’s Father and which entries be left blank.
The Court hereby sets the initial hearing on 24 January 2012 at 8:30 o’clock in the morning at which date, time and place, any interested party may appear and show cause why the said petition should not be granted.
WHEREFORE, let copy of this order be published at the expense of the petitioner once a week for three (3) consecutive weeks in a newspaper of general circulation in the Province of Pangasinan, including the cities of Dagupan, San Carlos, Urdaneta and Alaminos.
Furnish copies of this Order to the Office of the Solicitor General, Local Civil Registrar and the Provincial Prosecutor, Lingayen, Pangasinan.
