ORDER

This is a verified petition filed by James Julian Samelo Dudang praying for a judgment  changing his name from “Samelo” to “James Julian Samelo Dudang” and directing the Civil Registry and the National Statistics Office (NSO) to record the said change in the petitioner’s Certificate of Live Birth.

Finding the petition sufficient in form and substance, the same is set for initial hearing on November 23, 2015 at 8:30 o’clock in the morning, at which time and date, all persons having any opposition thereto my appear before this Court and show cause, if any, why the petition should not be granted.

Let this Order be published at the expense of the petitioner, once a week for three (3) consecutive weeks in a newspaper of general circulation in the province of Pangasinan, including the cities of Dagupan, San Carlos, Urdaneta and Alaminos, to be selected on the basis of a raffle.

SO ORDERED.
