ORDER

This is a verified petition filed by Joanna B. Quitaleg praying for the correction of the alleged erroneous entries in her record of birth bearing Registry No. 88-492, particularly as follows:

a- Year of Birth – From 1978

To: 1987;

b- Surname of the Child-From: Quitalig

To: Quitaleg;

c: Name of the Father – From: Trumfo C. Quitalig

To: Triumfo C. Quitaleg.

Finding the petition to be sufficient in form and substance, the same is set for initial hearing on October 20, 2014 at 8:30 o’clock in the morning, at which timne, and date, all persons having any opposition thereto may appear before this court and show cause, if any, why the petition should not be granted.

Let a copy of this Order be published at the expense of the petitioner, once a week for three (3) consecutive weeks in a newspaper of general circulation in the Province of Pangasinan, including the cities of San Carlos, Dagupan, Alaminos and Urdaneta, which shall be selected on the basis of a raffle.

Furnish copies of this Order to the Office of the Local Civil Registrar, Bayambang, Pangasinan, Office of the Solicitor General, Makati City and the Office of the Provincial Prosecutor, San Carlos City, Pangasinan.
