Before this Court is a verified petition for correction of  entry in the Certificate of live birth of WENRY E.B.G. SISON. Finding  the petition to be sufficient in form  and substance, let the initial hearing be set on DECEMBER 11, 2014 at 9:00 o’clock in the morning before the sala of this Court.

The petitioner is ordered to cause the publication of this Order in a newspaper of general circulation once a week for three consecutive weeks prior to the date set for hearing.

Notify the parties accordingly.

Given this 18^th day of August  2014, at Alaminos City,  Pangasinan.
