Petitioner seeks the correction of entry in his record of birth with the Local Civil Registrar of Mangatarem, Pangasinan specifically, as to date of birth from “JUNE 23, 1961” to “JUNE 23, 1959”;

The Court hereby set the initial hearing on  10 JUNE 2014 at 8:30 o’clock in the morning at which date, time and place, any interested party may appear and show cause why the said petition should not be granted.

WHEREFORE, let copy of this order be published at the expense of the petitioner once a week for three (3) consecutive weeks in a newspaper of general circulation in the Province of Pangasinan, including the cities of Dagupan, San Carlos, Urdaneta and Alaminos.

Furnish copies of this Order to the Cities of the Solicitor General, Local Civil Registrar and the Provincial Prosecutor, Lingayen, Pangasinan.
