Notice is hereby given that the estate left by the late FRANK BERMUDEZ, who died intestate on April 13, 2009, consisting of a Savings Deposit Accounts with the Philippine Veterans Bank, La Union Branch, under Savings Account No. 3215-80314-6 and 321502689-8, are the subject of affidavit of Claim with Waiver of Rights in favor of EVANGELINE BERMUDEZ BUSTO.
