ORDER

This is a verified petition filed by the petitioner, thru his counsel praying for the change of his name in his Certificate of Live Birth on file with the Civil Registry of Dagupan City from “KIMBERLY JOHN” and as to his gender from “Female” to “Male”.

Let the petition be set for hearing on January 26, 2012 at 8:30 o’clock in the morning on which date, time and place, all persons having any opposition thereto may appear and show cause, if any why the petition should not be granted.

Let this order be published at the expense of the petitioner once a week for three (3) consecutive weeks in a newspaper of general circulation in the Province of Pangasinan including the cities of Dagupan, Urdaneta, San Carlos and Alaminos which shall be selected on the basis of raffle.

Send a copy of this order and petition to the Civil Registry of Dagupan City, the Solicitor General, Makati City and the City of Prosecution Office, Dagupan City.
