ORDER

In a verified Petition for Change of Name and Correction of Entry in the Certificate of Live Birth of DOMMY VIRAY NARVAL, petitioner through counsel alleged inter-alia: that she is of legal age, single and a resident of Bilibid Caluttit, Bontoc, Mountain Province; that petitioner recently obtained his Certificate of Live Birth wherein she discovered some clerical errors, namely:

a. Name of Child: Dommy

b. Sex of child: Male

c. Date of Birth: May 5, 1957

that said clerical errors must be corrected to read as follows:

a. Name of Child: Dominga

b. Sex of Child : Female

c. Date of Birth: April 21, 1958

Finding the petition to be sufficient in form and substance, let the initial hearing be set on December 12, 2012 at 9:00 o’clock in the morning before Branch 55, Regional Trial Court, Alaminos City, Pangasinan at which time, date and place any interested person may appear and show cause why petition should not be granted.

Let this Order be published at the expense of the petitioner once a week for three (3) consecutive weeks in a newspaper of general circulation in the province of Pangasinan including the cities of Dagupan, San Carlos, Urdaneta and Alaminos.

Likewise, let copies of this Order, petition and annexes be furnished the Hon. Solicitor General at 134 Amorsolo St., Legaspi Village, Makati City and the City Civil Registrar of Alaminos City, Pangasinan.
