RESOLUTION

Finding the Amended Petition filed by the petitioner praying that respondent Local Civil Registrar of Dagupan City be ordered to correct his Middle Name from RICO to MEJIA and reflect the names of his true parents namely NENITA SORIANO MEJIA and NELSON RICO BAUTISTA, and direct the National Statistics Office to issue a Certificate of Live Birth reflecting his correct Middle Name and the names of his true parents pursuant to his Certificate of Live Birth registered on August 5, 2004, to be sufficient in form and substance, let the said amended petition be set for hearing on May 12, 2014 at 9:30 in the morning before this Court at which date, time and place, any interested person may appear and show cause why the petition should not be granted.

Let this Resolution and the amended petition be published once a week for three (3) consecutive weeks in a newspaper of general circulation in the province of Pangasinan, including the Cities of Dagupan, San Carlos, Urdaneta and Alaminos, at the expense of the petitioner.

Furnish copies of this Resolution to the City Civil Registrar of Dagupan Office of the Solicitor General, Makati City, Atty. J. Netu Tamayo, the respondents and the petitioner.
