ORDER

This is a verified petition dated September 19, 2011 filed by the petitioner through counsel for the correction of entries in the certificate of live birth of the petitioner re: gender from male to female and first name from Eduveges to Edoveges with the Municipal Civil Registrar of Malasiqui, Pangasinan.

Finding the petition to be sufficient in form and substance, the initial hearing of this case is herby set on December 5, 2011 at 8:30 in the morning, at which time, date and place, all persons having any opposition thereto may appear and show cause, if any, why the petition should not be granted.

Let a copy of this Order be published at the expense of the petitioner once a week for three (3) consecutive weeks in a newspaper of general circulation in the Province of Pangasinan including the Cities of Dagupan, San Carlos, Urdaneta and Alaminos which shall be selected on the basis of raffle.

Furnish a copy of the Order to the Offices of the Municipal Civil Registrar of Basista, Pangasinan, the Solicitor General, Makati City and the Office of the Provincial Prosecutor, San Carlos City, Pangasinan.
